Jim, I don't think any exhaust companies make a specific kit for those headers. Our customers commonly buy 2 stainless 30 or 45 degree mandrel bends and 2 stainless 3 bolt flanges. Before you weld the flanges on, line everything up so the lower leg of the pipe is parallel to the ground and at the desired height. Then you can always mitre cut the flange end of the pipe for final adjustment before welding the flanges on to the upper part of the pipe.
Also, for better performance, you can take the mitre'd bends to a local exhaust shop and have them flare the pipes just a bit to match up with the hole size of the flanges.
